RN Care Manager - PACE at Providence Health and Services
summary: Registered Nurse Care Manager for Providence PACE provides
comprehensive geriatric nursing including assessment, care
planning, implementation, coordination and ongoing monitoring to
keep older adults healthy and independent in the community. The
role involves interdisciplinary team facilitation, intensive
post-discharge medical management, home visits and clinic-based
care, and close collaboration with social workers and physicians.
Requires active RN licensure, at least one year of
long-term/geriatric nursing experience, with preferred BSN and
med-surg or rehab experience; full-time, on-site day position with
a hiring bonus and competitive benefits. Description $15,000 hiring
bonus for eligible external candidates that meet required
qualifications and conditions for payment. The Registered Nurse
Care Manager establishes a well-rounded care practice that
includes: a patient assessment, care planning, implementation,
coordination, and monitoring and ongoing routine evaluation that
ensures medically and financially appropriate care.
Responsibilities include: facilitating interdisciplinary team care
coordination, intensive medical management of participants recently
discharged from hospital and skilled nursing facilities.
Additionally, this position will work closely with the IDT Social
Worker and Physician to collect necessary medical and social
information to ensure comprehensive decisions are made which ensure
the safety and well being of participants. Providence PACE is a

Qualifications: Education sufficient for clinical licensure Upon
hire: Washington Registered Nurse License, or license in states
wherever care is delivered 1 year long term care or geriatric
nursing. Preferred Qualifications: Bachelor’s Degree in Nursing

for this position. The successful candidate's placement on the wage
range for this position will be determined based upon relevant job
experience and other applicable factors. These amounts are the base
pay range; additional compensation may be available for this role,
such as shift differentials, standby/on-call, overtime, premiums,
